Who We Are Seeking... Bridges Outreach is seeking a compassionate, motivated, and experienced Housing Stabilization Case Manager to join our Newark Project Connect Team. The ideal candidate is a strong communicator and problem-solver who can work independently and collaboratively while exercising sound, professional judgment. This role works directly in the field with individuals experiencing homelessness and those transitioning to and maintaining permanent housing.
Responsibilities:
The Housing Stabilization Case Manager (HSCM) provides comprehensive, housing-focused case management to individuals seeking and maintaining permanent supportive housing.
The HSCM will:
Conduct housing-first assessments to identify participants' strengths, resources, and barriers to stable housing. Develop individualized service and housing stability plans; monitor and evaluate progress. Provide at least weekly case management and additional support as needed. Assist participants with housing applications, documentation, benefits, income, budgeting, credit repair, and other resources necessary to achieve housing stability. Research affordable and subsidized housing opportunities and assist participants in securing appropriate permanent housing. Develop and maintain relationships with landlords, housing authorities, and community partners. Support participants through the housing transition, including move-in, inspections, lease requirements, and establishing plans for rent payments. Provide ongoing case management following housing placement to promote long-term housing stability and community connection. Conduct home visits and address challenges related to maintaining a home, including budgeting, relationships, visitors, isolation, and household responsibilities. Assist with annual housing recertifications and maintaining or increasing household income and benefits, including SNAP. Connect participants to mental health, physical health, substance use, and other community-based services; provide ongoing referrals, advocacy, and support. Maintain accurate and timely case records, including service plans, case notes, assessments, and required program data in Salesforce, HMIS, and other systems. Compile data and contribute to reports and performance metrics. Participate in weekly case conferences, staff meetings, trainings, and relevant Newark/Essex County community meetings. Perform other duties as assigned to support participant and housing stability.
Required Qualifications:
Bachelor's degree in Social Work or a related field, or 3-5 years of relevant case management experience in a health or social service setting; experience serving individuals experiencing homelessness preferred. Experience working with underserved populations, including individuals experiencing mental health or substance use challenges, preferred. Strong written, verbal, interpersonal, organizational, and time-management skills. Demonstrated cultural competence, humility, and ability to work effectively with people from diverse backgrounds and lived experiences. Strong attention to detail, documentation, follow-through, and ability to manage multiple priorities. Proficiency with Google Workspace and Microsoft Office; experience with HMIS preferred. Valid driver's license and safe driving record. Ability to travel throughout the service area; 50%+ travel required . Mileage reimbursement or a company vehicle is available when needed. Ability to lift up to 35 pounds.
Preferred Qualifications:
Motivational Interviewing and/or Trauma-Informed Care experience. SOAR Certification. Crisis de-escalation certification. Bilingual English/Spanish.
