Case Manager 035 - Please Apply Using Job Code 21253 X Pinterest September 4 , 2026 Catholic Charities, Diocese of Metuchen (CCDOM), with more than 450 employees is a non-profit, social services agency with its mission, driven by Catholic social teaching, of providing quality services with dignity and respect to the poor, vulnerable and all people in need and partnering with families and communities to improve the quality of life. We serve more than 68,000 clients per year throughout Middlesex, Somerset, Hunterdon, and Warren counties in NJ. Case Manager 035 -
New Brunswick NJ Salary:
$45,000 per year + $2,500 sign on bonus
Key Responsibilities:
Provide case management services to individuals residing in the Ozanam Inn Shelter. Screening, assessment, service planning, linkage, crisis intervention, and de-escalation, managing relationships with a broad range of community providers, and life skills teaching. Transporting individuals to housing opportunities, assisting client in applying for housing, building connection with other agencies and utilization of HMIS (WellSky) case management software and MyEvolv. Requires on-call coverage and evening hours. Bi-lingual (Spanish) encouraged to apply. Ability to work independently.
Requirements:
Bachelor's degree (B.A), preferably in Social Work or a similar Human Service field is preferred. Experience with conflict resolution and mediation. At least two (2) years relevant work experience and demonstrated ability to provide effective services to clients required. Must have valid driver's license with acceptable driving history.
