Shelter Case Manager

Job Description

Position Summary The Family Shelter Case Manager is responsible for providing comprehensive, trauma-informed case management to families with children residing in Family Promise of the Jersey Shore's 24/7 Family Shelter. The primary goal of this position is to help families move from homelessness to safe, stable, permanent housing as quickly as possible. The Case Manager works intensively with each family to identify barriers, develop individualized Housing Stability Plans, connect families to community resources and benefits, increase income and employment when appropriate, locate permanent housing, and prepare families for long-term housing stability. The Case Manager works closely with the Case Manager Supervisor, Shelter Supervisor , shelter Case Aides, Family Promise leadership, partner agencies, landlords, DCPP/DCF when applicable, schools, healthcare providers, and community resources to ensure coordinated services for each family. This is a hands-on position requiring strong communication, documentation, professional boundaries, crisis-management skills, and the ability to work effectively with families experiencing significant housing and financial instability. Essential Duties and Responsibilities Comprehensive Case Management Conduct comprehensive intake and assessment with each family entering the Shelter. Review the family's immediate safety, housing, financial, employment, transportation, childcare, educational, medical, behavioral health, and other service needs. Identify the circumstances that contributed to the family's homelessness and barriers that may prevent the family from obtaining or maintaining permanent housing. Develop an individualized Housing Stability Plan with each household. Establish specific, measurable, and realistic goals with each family. Meet with shelter families regularly and maintain consistent case management contact throughout their stay. Monitor progress toward established goals and adjust plans as circumstances change. Maintain a strong focus on permanent housing throughout the family's shelter stay. Help families understand that shelter is temporary and that active participation in case management and housing planning is essential. Advocate for clients while simultaneously promoting accountability, independence, and self-sufficiency. Housing Search and Housing Stabilization The Case Manager will actively assist families with identifying and securing permanent housing, including: Searching for appropriate rental opportunities. Helping clients complete housing applications. Assisting clients with gathering identification and required housing documentation. Communicating with landlords and property managers. Advocating for families experiencing barriers to obtaining housing. Helping clients address prior evictions, rental debt, poor credit, insufficient income, or other housing barriers. Connecting families with affordable housing opportunities, rental assistance, rapid rehousing, prevention/diversion programs, housing authorities, and other available resources. Coordinating security deposits, rental assistance, landlord incentives, or other financial assistance when approved and available. Ensuring that no financial commitments or agreements are made with landlords or vendors without required supervisory approval. Helping families understand leases, tenant responsibilities, budgeting, and expectations associated with maintaining permanent housing. Developing a discharge and stabilization plan before the family exits shelter. Providing or coordinating follow-up services after housing placement when applicable. Income, Employment and Financial Stability The Case Manager will work with families to establish a realistic plan for long-term financial stability.
Responsibilities include:
Reviewing all household income and benefits. Assisting clients with employment searches when appropriate. Helping clients develop resumes and complete employment applications. Connecting clients with workforce development, vocational training, educational opportunities, and employment programs. Assisting eligible families with applications for mainstream benefits. Helping clients access SNAP, TANF, SSI/SSDI, unemployment, childcare assistance, Medicaid/NJ FamilyCare, and other benefits when appropriate. Developing realistic household budgets with families. Helping families understand anticipated rent, utilities, transportation, food, childcare, and other expenses. Identifying financial barriers that could jeopardize future housing stability. Encouraging savings and responsible financial planning whenever possible. Children and Family Needs Because this is a family shelter, the Case Manager will consider the needs of both the parent/caregiver and children within the household.
Responsibilities may include:
Coordinating with schools to minimize educational disruption. Helping families address transportation to school. Connecting families with childcare resources. Identifying developmental, educational, behavioral, or emotional support needs. Connecting families with appropriate youth and family services. Supporting parents in maintaining healthy routines and stability for their children while residing in shelter. Coordinating with
DCPP/DCF
when applicable and appropriately authorized. Advocating for the best interests, safety, and stability of children while maintaining appropriate confidentiality. Health, Behavioral Health and Recovery Supports The Case Manager will assess whether medical, behavioral health, substance use, or other wellness concerns create barriers to housing stability.
The Case Manager will:
Connect families to appropriate medical and behavioral health providers. Assist clients with obtaining health insurance or accessing healthcare resources when necessary. Provide referrals for substance use treatment or recovery supports when appropriate. Coordinate with outside providers with appropriate client authorization. Utilize trauma-informed, person-centered practices. Maintain professional boundaries and recognize when a client's needs require specialized clinical intervention beyond the Case Manager's scope. Documentation and HMIS Accurate and timely documentation is an essential function of this position.
The Case Manager must:
Complete all required intake documentation. Enter and maintain accurate client information in HMIS. Complete case notes following every significant client interaction. Document referrals, services, housing searches, landlord communication, financial assistance, goals, progress, and outcomes. Keep Housing Stability Plans current. Maintain organized electronic and/or physical client files. Obtain appropriate releases of information. Maintain documentation required by government contracts, grants, DCPP/DCF, and other funding sources when applicable. Ensure case notes are completed within established agency timelines. Maintain client confidentiality in accordance with Family Promise policies and applicable laws and regulations.
Qualifications Required/Preferred Qualifications:
Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Criminal Justice, or a related field preferred; equivalent relevant experience may be considered. Experience providing case management, housing services, homeless services, family services, behavioral health services, or other human services strongly preferred. Experience working with families experiencing homelessness or housing instability preferred. Knowledge of Ocean County resources is highly desirable. Strong written and verbal communication skills. Strong organizational and documentation skills. Ability to manage multiple families and competing priorities. Ability to maintain professional boundaries. Ability to work independently while functioning as part of a multidisciplinary team. Ability to communicate effectively with individuals from diverse backgrounds and circumstances. Ability to remain calm and professional during crisis situations. Basic computer proficiency and ability to learn HMIS and other required systems. Valid driver's license and reliable transportation as required for the position. Ability to successfully complete all required background checks and training. Schedule and Availability This is a full-time position within a 24-hour, 7-day-per-week family shelter environment. The Case Manager's primary schedule will be established by Family Promise based upon program needs. Because shelter operations continue evenings, weekends, and holidays, occasional schedule flexibility may be required for family emergencies, intakes, housing placements, meetings, case conferences, or other critical program needs. The Case Manager is expected to communicate promptly with the Case Manager Supervisor and Shelter Supervisor regarding emergencies, absences, client concerns, or situations requiring supervisory involvement.
