As a Mi Via Consultant / Support Broker, you will work directly with participants and their families to help them successfully navigate the Mi Via program and make informed decisions about their services. The role entails oversight of budget services, documentation assistance, calls/visits to check in with members and review satisfaction with services, travel to visits, filing, and the ability to meet deadlines timely. Responsibilities/ Requirements Bachelor's degree in Social Work, Psychology, Special Education, Counseling, or a related human services field is strongly preferred (equivalent professional experience in the developmental disabilities waiver field may substitute). Building trusting, professional relationships with Mi Via participants and their families. Helping participants understand their Mi Via services and self-directed options. Assisting participants with person-centered planning and identifying their individual goals and needs. Helping participants develop and maintain their service plans. Supporting participants in choosing services, supports, providers, and community resources. Helping participants understand and manage their responsibilities within the self-directed model. Conducting monthly visits with participants and their supports to ensure utilization and service requirements are being met. Coordinating with families, providers, employers, fiscal/intermediary entities, and other members of the participant's support team. Monitoring services and checking in with participants to ensure their needs are being addressed. Helping participants identify solutions when they encounter challenges or barriers. Advocating for participant choice, independence, dignity, and self-determination. Maintaining accurate case notes and required documentation. Responding appropriately to changes in participant needs or circumstances. Connecting participants with community resources and other supports. Maintaining confidentiality and following program and agency requirements.
