Job Description

Casa Cerrillos Case Management 1) Provides case management to Casa Cerrillos clients. This includes initial assessment, development of individual service plans, regular meetings with clients, arrangement for and attendance at meetings with important referral resources, staffing each case as needed with other St. Elizabeth Shelter staff members and outside professional resources and other services. 2) Co-facilitates and facilitates specific group sessions for clients enrolled in the Casa Cerrillos Program. 3) Assists in assessing clients for appropriateness for referral into the Casa Cerrillos Program from other St. Elizabeth Shelter programs or outside agencies. 4) Assists other St. Elizabeth Shelter staff members in developing or revising agency standards for case management, client services, record keeping, discharge planning and other program-related functions. 5) Provides in-service training and education to St. Elizabeth Shelter staff members on specific topics related to case management or other areas of expertise.

Meets, as needed, with staff of other agencies to develop or enhance agreements and protocol for referrals and services to St. Elizabeth Shelter clients. 6) Attends regular staffing meetings with the case management team to review specific clients, overall protocols, issues and areas needing improvement. Participates in corrective action plans for individual clients and agency protocols and, if applicable, as the result of site visits. 7) Participates in special projects initiated by St. Elizabeth Shelter related to direct services and programs. 8) Performs other tasks as required to provide effective case management services to St. Elizabeth Shelter clients. 9) Shares on-call duties with other Casa Cerrillos staff Experience and Skills Bachelors degree in Social Work (or equivalent) preferred Spanish speaking preferred Experience working with homeless individuals and/or families preferred Willing and able to work with diverse populations Effective communication skills Computer proficient Knowledge of the unique cultural heritage of northern
