The Senior Social Services Coordinator will: Provide technical assistance to agencies to be educated and using a person-centered focus; Provide support to those individuals receiving Day and Employment Services and to support those Agencies that provide Day and Employment Services. COVID has intensified the need for Community Inclusion Coordinators (CICs) technical assistance and support. How does it get done? The Senior Social Services Coordinator is responsible for: Attending interdisciplinary team meetings to include assisting the team with problem solving and mediating conflict resolution within the team; Monitoring service provider compliance and follow up on litigation activities in the Northeast Region; Phone calls with teams, providers, family members, guardians, advocates and individuals that are inquiring about re-opening of services and when congregate services may return; Coordinating with Division of Vocational Rehabilitation to support people with disabilities to work; Working with Northern New Mexico Schools to support family to apply for the DDSD Wavier programs; Completing assigned safety home visits 2 times a year for the caseload. Who are the customers? This position assists with people receiving services through the Developmental Delay Waiver and Mi Via Waiver as well as people transitioning between waivers. To support the caretakers to obtain what they need to keep the individual safe. Ideal Candidate This position assists with people receiving services through the Developmental Delay Waiver and Mi Via Waiver as well as people transitioning between waivers. Minimum Qualification Associate's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology, Family Studies/Services, Social Sciences, or Human Services and one (1) year of experience as a social work assistant and/or in social or community coordination, working with communities, working on health or social service related matters, social work/case management experience, and/or behavioral health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling three (3) years may substitute for the required education and experience. Employment Requirements Must possess and maintain a valid Driver's License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive a Defensive Driving Course Certificate as a condition of continued employment within 45 days of employment. Travel may be required with some occasional overnight stays. Successful completion of the Health Insurance Portability and Accountability Act (HIPAA). Working Conditions Work is performed in an office setting: late hours and weekends work may be required. Will be exposed to regular periods of video display terminal and keyboard usage and stressful situations. Travel may be required. Incumbent will work under stress and frequent time constraints.
