Job Description

JOB PURPOSE
The DHRC Case Manager is responsible for providing comprehensive case management services to individuals and families in need of supportive services, and community resources. This role focuses on assisting clients in achieving stability and self-sufficiency by creating individualized service plans, connecting them to essential resources, and offering ongoing support.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Client Services and Case Management Provide immediate support and intervention during client crises. Develop safety plans and connect clients to emergency services when necessary. Conduct regular monthly assessments with clients to ensure they are receiving the necessary assistance and support. Collaborate with the workforce and other case managers. Collaborate with supervisor to resolve complex issues and create comprehensive plans. Able to prioritize multiple client needs in a fast paced environment Collaborate with internal and external case managers and supervisors to ensure clients receive all possible services, resolve complex cases, and develop creative solutions to challenges. Resource Coordination and Support Services Maintain up to date knowledge of available community resources and programs. Mediate issues with clients that threaten their self sustainability. Assist clients in developing and maintaining budget plans to ensure stable self reliance. Provide guidance on financial literacy, budgeting, saving and debt management. Documentation and Reporting Maintain accurate and comprehensive case notes, documenting client status, progress, assessments, service engagement and any relevant correspondence. Complete necessary forms and reports following established procedures. Diligently and accurately record all provided services, referrals, recommendations, case notes, and program documentation into salesforce within 1 business day of client interaction. Ensure shared spreadsheets are updated within 1 business day of any changes. Prepare and submit weekly success stories to supervisor, marketing and grants manager. Performs other related duties consistent with the scope and purpose of the position.

Proficiency in using client management systems to document case notes, track referrals, and maintain program compliance

Ability to perform accurate data entry and track program metrics
Service Planning:
Ability to reevaluate service plans and adapt strategies to improve client outcomes
Resource Navigation:
Knowledge of community resources to address barriers and provide appropriate referrals
Financial Guidance:
Ability to use budgeting tools to support client financial planning
Technical Skills:
Proficiency in Microsoft Office Suite Word, Excel, PowerPoint and Google Workspace including
Gmail, Drive, Docs, Sheets, Slides, Google Meet and Calendar Crisis Response:
Ability to provide crisis stabilization and rapid response
Professional Standards:
Ability to demonstrate professionalism, teamwork, accountability, and a trauma informed approach
EDUCATION AND EXPERIENCE
Minimum Qualifications Bachelor's degree in Social Work, Human Services, or a related field, or an equivalent combination of education and relevant work experience in workforce case management Minimum 2 years of Non Profit experience Minimum 3 years of experience in case management, social services, and community outreach Experience working directly with diverse populations, including vulnerable or at risk individuals Must obtain and maintain a valid Nevada Food Handlers Card within thirty days of hire due to required participation in food distribution events. Preferred Qualifications Relevant certifications in case management and conflict resolution Knowledge of employment related issues, housing programs, and health resources Familiarity with housing services, wrap around programs, and self sufficiency initiatives Proven track record of managing a caseload and delivering measurable outcomes in client stability and self reliance
WORK SCHEDULE AND CLASSIFICATION
This position is full time, requiring approximately 40 hours per week The standard work schedule is Monday through Friday, 11:30 a.m. to 8:00 p.m. Occasional Saturday coverage may be required based on business needs, with hours of 8:30 a.m. to 5:00 p.m. This role is classified as non-exempt under the Fair Labor Standards Act
TRAVEL REQUIREMENTS
If the position requires travel to perform essential job duties, the employee must maintain a valid driver's license and meet the organization's insurability requirements. Reliable transportation is required to fulfill travel related responsibilities.
WORK ENVIRONMENT AND PHYSICAL DEMANDS
Ability to sit, stand, and walk for extended periods throughout the workday. Ability to lift and carry up to 50 pounds as needed. Comfortable working in both office and community-based settings, including occasional outdoor exposure. Ability to operate office equipment, computer systems, and telephone.
