Established in 1926, Family Service League is a social services agency transforming lives and communities through mental health, addiction, housing, and essential human services with more than 60 social service programs in over 20 locations.
SCHEDULE
Flexible schedule: Five days - 35 hour work week, Availability to work 2 evenings a week required.
SUMMARY:
Family Service League is seeking a full-time Case Manager for the Alternatives for Youth Program. The Case Manager will coordinate in home family support designed to divert involvement in the Suffolk County Probation System with its philosophy founded on empowering parents/guardians and equipping families for success and restoration. The Case Manager will conduct visits in the clients' home and provide direct services to the youth. RESPONSIBILITIES
The Case Manager will provide linkages, coordination, advocacy and mentoring. Engage families in conflict management and skill building. Provide direct services, including anger management and parenting skills to youth and parents in order to avoid involvement with the juvenile justice system. The Case Manager will conduct assessments and maintain electronic health records of all services provided. Conduct home visits throughout Suffolk County. All other duties as assigned.
QUALIFICATIONS
Bachelor's degree, or Associate's degree with one years of human service experience or High School Diploma with three years of human service experience required. At least one year experience working with community resources is required. Excellent interpersonal, organizational, and problem-solving skills required. Strong communication skills, verbal and written, are required. Ability to work as part of a team. Proficient computer skills, including Microsoft Office, required. Valid and clean NYS Driver's license required. Travel required.
PHYSICAL REQUIREMENTS
This position requires sitting for long periods of time and travelling throughout Suffolk County to conduct home visits.
