Buffalo, NY Job Details From $17.47 an hour 2 hours ago Qualifications Passenger vehicle High school diploma or GED Driver's License Full Job Description Provides education, training, assistance and advocacy to people within the Alternatives to Incarceration (ATI). Completes scheduling and attends various appointments with program participants such as medical, educational, court and financial appointments. Completes administrative provisions of service delivery in ATI programs in order to support persons receiving services.
POSITION RESPONSIBILITIES 1.
Schedules initial and follow up appointments for necessary supports and services, including those related to medical, educational, financial, and parenting. 2. Provides assistance and advocacy during appointments for requested services. 3. Provides assistance with court mandated services. 4. Arranges and or provides transportation and accompanies individual to appointments as needed in conjunction with program services. 5. Secures necessary reports and/or health care information from other providers. 6. Conducts research for resources based on identified needs. 7. Provides Moral Reconation Therapy (MRT) for ATI clients via individual sessions with clients and co-facilitating group MRT sessions with the ATI Assessor and Court Liaison. 8. Assists the ATI Court Assessor and Court Liaison with implementing Interactive Journaling with ATI clients as needed. 9. Maintains required individual records including case notes, in compliance with funding source documentation requirements (DCJS). 10. Assists with case record maintenance, including purging and filing of paperwork. 11. Follows mandated reporting guidelines. 12. Participates in specific staff meetings, in-service trainings, and supervision, including clinical supervision with the Specialized Services Dept.
SUPERVISORY RESPONSIBILITIES
This position has no supervisory responsibilities.
WORKING CONDITIONS
1. Will usually work normal business hours. May need to work outside of normal business hours to accommodate people's appointments and needs. 2. Local travel required approximately 75% of the time. 3. Provide services to individuals in their homes as well as in the community. 1. Education and Experience a. High School Diploma/GED 2. Skills, Knowledge and Abilities a. Knowledge of developmental disabilities and service planning. b. Good oral and written communication skills and basic computer skills. c. Must be able to read and write case notes as well as understand written directions. 3. Certifications - Must be or become a. CPR/First Aid certified. b. Must complete the SCIP Philosophy section of the SCIP-R curriculum . Additional SCIP training or certification may be required based on participant needs. 4. Verifications and Screenings a. OPWDD clearance for convictions of criminal offenses. b. Clearance through NYS Central Child Abuse and Maltreatment Register. c. A drug screen prior to the commencement of employment. d. Approval of NYS Justice Center. 5. Be authorized as an Agency Driver by a. Holding and maintaining a valid NYS Driver's License. b. Maintaining an approved driving record according to Agency Policy and Procedure. 6. Have and maintain have use of personal vehicle for work purposes; travel between work locations and for transporting people served a. Proof of insurance on personal vehicle being used for work purposes. 7. Physical and Mental Requirements a. Seeing, hearing, walking, sitting, standing, driving, stooping, bending, carrying, stretching, and climbing stairs. b. Lifting up to thirty (30) pounds. c. Exercising independent judgment, concentrating, thinking, communicating, interacting with others, managing multiple tasks and coping with stressful situations and deadlines. d.
