Job Description

SOUTH BUFFALO COMMUNITY ASSOCIATION JOB DESCRIPTION - CASE MANAGER GENERAL PURPOSE
To provide case management, outreach, information, and referral to senior citizens primarily in South Buffalo, Lackawanna, Blasdell, Orchard Park, and West Seneca. The target population is frail, high-risk elderly individuals.
REQUIREMENTS
Bachelor of Arts or Bachelor of Science degree, preferably in the Human Services field from an accredited regional, national, or specialized agency recognized as an accrediting agency by the U.S. Department of Education/U.S. Secretary of Education. Related job experience with the elderly is a plus. Experience with Excel, Word, or Google Workspace. A car is necessary as traveling to clients' homes is expected.
DUTIES:
Conduct assessments of needs and finances during routine home visits; develop, implement, and monitor care plans in accordance with County and State regulations; and provide case management when appropriate. Assist clients with completing entitlement forms such as
HEAP, SNAP
(food stamps), and Medicaid. Assist clients with applying for or continuing services such as: Home-delivered meal options, Home Care services, Transportation, and Personal Emergency Response units. Maintain updated client case records utilizing appropriate entry systems, including daily computer data entries. Other related duties as deemed appropriate by the Senior Services Project Coordinator and/or Executive Director.
SUPERVISION/ACCOUNTABILITY
The Case Manager is supervised by and is accountable to the Senior Services Project Coordinator and Executive Director.

Pay Rate:
