Job Description

JOB DESCRIPTION
Under the general supervision of the Shelter Manager (SM), the Social Care Network Screener - Navigator is responsible for screening Medicaid Members for their social care needs; conducting social care navigation, including eligibility assessment and referral to supportive programs; and providing certain enhanced services to meet individuals' needs. This position will support the HONOR's role in the Social Care Network by focusing on the screening and navigation activities. This position will screen Medicaid Members to identify their social care needs using a standardized screening tool, then carry out eligibility assessments to confirm Members' eligibility for the Social Care Network. Depending on eligibility and need, Members may be referred to Social Care Network programs or to other local, state, and federal programs. The Social Care Navigator will collaborate with other HONOR staff, as well as other SCN participating Agencies across the Hudson Valley Region.
QUALIFICATIONS FOR THE POSITION
(Not the Individuals Qualifications) 1. Bachelor's Degree in Health and Human Services or related field. 2. One year experience in eligibility screening, social services, or healthcare preferred or Community Health Worker. 3. Ability to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people. 4. Strong interpersonal and communication skills are essential for building rapport and trust with individuals 5. Integrity, positive attitude, mission-driven, and self-directed with demonstrated passion for HONOR's mission. 6. Must have and maintain a valid New York State driver's license. 7. Willingness to travel to other off sites locations. 8. Detail-oriented and have competent literacy and writing, documentation, communications and interpersonal abilities (friendly, courteous, helpful, ability to work as part of a team). 9. Bilingual preferred.
Pay:
$20.00 - $23.00 per hour
