Job Description

We are seeking an experienced, compassionate, and highly organized Case Manager to join our leadership support team at our busy Westbury assisted living community. This role is ideal for someone who thrives in a fast-paced environment, enjoys building relationships with seniors and families, and is passionate about improving the quality of life for older adults. This is not a desk-only role. We are looking for someone who is hands-on, resident-focused, emotionally intelligent, and able to collaborate closely with families, nursing staff, administration, activities, and community resources. What You'll Do As a Case Manager, you will play a critical role in supporting residents and their families throughout their journey within our community.
Responsibilities include:
Supporting the admission and transition process for new residents Conducting resident assessments and maintaining case management documentation Building strong relationships with residents and families Assisting residents with adjustment, emotional support, and social wellbeing Coordinating services and resources both inside and outside the facility Monitoring changes in resident condition and communicating concerns appropriately Assisting with hospital transfers, appointments, and continuity of care Supporting resident advocacy, resident council, and quality of life initiatives Collaborating with interdisciplinary teams to ensure resident needs are met Maintaining accurate documentation in accordance with NYS Department of Health regulations Serving as a compassionate resource for residents experiencing cognitive or behavioral changes Participating in community events and resident engagement activities What We're Looking For The ideal candidate is: Compassionate and patient Highly organized and detail-oriented Comfortable working with seniors experiencing cognitive decline Able to manage sensitive family dynamics professionally A strong communicator and team player Comfortable working in a fast-paced assisted living environment Qualifications Bachelor's Degree in Human Services, Social Work, Psychology, Gerontology, or related field preferred Minimum 1 year of experience working with older adults or in healthcare/social services Experience in assisted living, long-term care, senior services, or case management strongly preferred Knowledge of NYS DOH regulations preferred Experience with documentation and resident assessments Ability to exercise sound judgment and maintain confidentiality Why Join Amber Court?
