Job Description

Job Summary We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting clients by assessing their needs, developing personalized care plans, and connecting them with appropriate resources. This position requires strong interpersonal skills and a commitment to improving the lives of individuals facing various challenges.
PLEASE NOTE
2nd shift 3pm-11pm Monday-Friday
MUST HAVE DRIVERS LICENSE AND VEHICLE
Highly prefer someone with CDCA or similar license Responsibilities Conduct comprehensive assessments to determine clients' needs and eligibility for services. Develop, implement, and monitor individualized care plans tailored to each client's unique situation. Provide crisis management and intervention services to clients in need of immediate support. Collaborate with community resources, including addiction counseling, behavioral health services, and workforce development programs. Assist clients with navigating Medicaid and housing applications. Maintain accurate documentation of client interactions and progress notes in accordance with agency policies. Advocate for clients within the community and ensure they receive necessary support services. Facilitate workshops or group sessions related to child welfare, childcare, and other relevant topics. Provide transportation to appointments and resources Work closely with housing staff to ensure compliance with programs Experience A background in social work or a related field is preferred. Experience in addiction counseling or behavioral health is highly desirable. Knowledge of crisis management techniques and experience in crisis intervention is essential. Previous experience working with diverse populations and understanding their unique challenges will be beneficial. Strong communication skills, both verbal and written, are necessary for effective client interaction.
