The Phoenix Center About Us:
TPC Wellness provides comprehensive care and support services to individuals on their recovery journey. We are seeking a dedicated and organized Case Manager to join our team. This role is essential in connecting individuals with resources, monitoring progress, and supporting long-term success. Position Overview The Case Manager works under the supervision of the Lead Case Manager to maintain caseloads, support individuals in the community and office, and ensure compliance with agency and regulatory standards. The ideal candidate will be compassionate, organized, and able to balance both direct support and documentation responsibilities. Key Responsibilities Maintain caseloads and ensure regular contact with individuals served. Monitor progress and assess needs in line with policies, contracts, and best practices. Meet with individuals one-on-one and in groups; collaborate with families, case managers, and clinicians. Provide crisis intervention, advocacy, assistance with ADLs, financial assistance, and community resource linkage. Visit, monitor, and transport individuals as needed. Prepare service plans and clinical reports using electronic health record (EHR) software. Maintain accurate and up-to-date documentation and records. Conduct family outreach and participate in the Loved Ones Group. Communicate with probation departments regarding client progress. Participate in trainings, team meetings, and contribute to a safe, supportive environment.
Qualifications Required:
Valid driver's license.
Education:
High school diploma (required). Bachelors degree in Human Services, Criminal Justice, or related field.
Certification:
Active CDCA (Chemical Dependency Counselor Assistant) required. Skills & Attributes Strong organizational and time management skills. Proficient with technology and EHR systems.
