Contract Veteran Directed Care (VDC) Case Manager Are you passionate about helping Veterans maintain independence and control over their care? We are seeking a Contract VDC Case Manager to support Veterans in the Veteran Directed Care program through person-centered planning, care coordination, and ongoing case management. Key Responsibilities Conduct assessments and home visits to identify needs and develop individualized care plans. Guide Veterans in managing personal care services, spending budgets, and caregiver arrangements. Complete monthly contacts, quarterly in-person visits, and annual reassessments. Coordinate with community partners and connect Veterans to available resources. Maintain accurate documentation and ensure program compliance. Qualifications Bachelor's degree in Human Services, Social Work, or related field preferred. Two or more years of case management or home and community-based services experience. Strong knowledge of social work principles, case management practices, and client communication strategies. Proficiency in Microsoft Office tools (Word, Excel, Outlook) for documentation and reporting purposes. Excellent writing skills for accurate record keeping and report generation. Ability to manage multiple cases efficiently while maintaining attention to detail through effective data entry skills. Knowledge of Veteran services, person-centered planning, or self-directed care preferred.
