We are professional, agile and our goal is to provide substance use treatment and residential living to low-income individuals with substance use issues. The Life Transformation Program is a faith-based, 12-step program lasting 9-12 months. Seeking case manager (male preferred) to work with male residents at Jesus House. Jesus House is an OKARR Level 3 certified recovery program for men and women dealing with substance use disorder.

Duties:

a) Provide case management services to individuals and families in need b) Conduct assessments to determine client needs and develop appropriate individual service plans c) Monitor client progress toward goals and adjust goals as needed d) Collaborate with community resources and agencies to coordinate services for clients e) Advocate for clients and ensure they have access to necessary resources and support f) Monitor client progress and make necessary adjustments to service plans g) Provide counseling and guidance to clients, utilizing motivational interviewing techniques h) Support clients in workforce development activities, such as job training and employment placement i) Mentor clients and provide guidance on personal and professional development j) Maintain case notes and complete required record keeping in electronic record k) Distribute medication to residents l) Receive referrals from other agencies and determine client appropriateness for Jesus House Program m) Assist residents with getting appropriate social services as needed n) Pay is salary; classification is "at will."

Requirements:

Bachelor's degree in social work, psychology, substance use or three years of experience in the human services field and current case management certification or a related field Experience working with diverse populations, including individuals with mental health and substance use issues Knowledge of various housing programs and eligibility requirements Familiarity with Medicaid/Sooner Care regulations and benefits Strong communication skills, both written and verbal Ability to work independently as well as part of a team Excellent organizational skills Ability to handle sensitive and confidential information with professionalism and discretion Ability to multitask and work in a fast-paced environment Deescalation skills a must Keen attention to detail and able work in a culture of excellence. Able to bend, stoop, move about the facility freely, climb stairs, lift 30 pounds and sit for extended periods of time Familiar with
