What You'll Do As an SSVF Case Manager, you will partner directly with veterans and their families to help them navigate housing challenges and access community resources that support long-term stability.
Key responsibilities include:
Managing a diverse caseload of veterans and their families. Conducting individualized assessments and identifying barriers to housing success. Developing personalized housing stability plans based on participant goals and needs. Coordinating services with community organizations, landlords, government agencies, and veteran-serving partners. Monitoring participant progress and adjusting support strategies as needed. Advocating for participants while promoting self-sufficiency and long-term success. Maintaining accurate and timely case documentation to support quality services and program compliance. Collaborating with internal teams and external stakeholders to achieve positive housing outcomes. What Makes You Successful We're looking for someone who is: Passionate about serving veterans and vulnerable populations. Skilled at building trust and rapport with individuals from diverse backgrounds. An effective communicator, both written and verbal. Organized and able to manage multiple priorities in a fast-paced environment. Comfortable working independently while also contributing to a collaborative team culture.
