JOB SUMMARY
In collaboration with the Landlord Tenant Liaison, this position is responsible for assisting clients living in the shelter to move from homelessness into permanent housing and to obtain the tools and resources they need to remain in permanent housing. The Housing Stability Case Manager will provide housing stability and income-focused case management services to the families.
SPECIFIC JOB DUTIES
Conduct the assessment households strengths and barriers to housing. Work with program participants to develop a strengths-based Housing Stabilization Plan. The plan may include goals related to childcare, employment, family reunification, financial stability, health, housing, legal issues, and other concerns as appropriate. The plan must include a goal to sustain housing following the end of program assistance, including either goals to increase income, reduce debt and/or decrease household expenses. The plan must be monitored and updated at regular intervals. A copy of the current plan must be given to the participant and kept in their file. In coordination with Landlord/tenant Liaison, assist participants identify and select housing options based on their unique needs, preferences and financial resources. When necessary, accompany families to view units, advocate for families with landlord and assists families with applications and other paperwork. Recognizing that program participants may be severely rent burdened at the end of our assistance, ensure that participating households receive only the assistance necessary to stabilize in permanent housing. Meets monthly or as needed with families on caseload to review progress and to follow-up on tasks and services. Tracks and documents residents' progress. Holds residents accountable for achieving goals and objectives Educate clients regarding tenant rights and responsibilities, housing discrimination, finding and securing rental housing, communication with landlords, fair housing and the eviction process. Exit program participants from the program when they are no longer imminently at-risk of becoming homeless in the near term. Negotiate and manage the Rapid Re-housing direct assistance budget in coordination with Director to ensure the distribution of funds is in keeping with state and local guidelines and meets the needs of clients. Documents, maintains and updates Homeless Management Information System (HMIS) to include up-to-date information on services provided Participates in staff meetings and presents information to team regarding client progress, including non-compliance and lack of progress, with possible solutions to ensure best support for the residents' success Attend agency, LVRHAB and community meetings as directed by supervisor. Continuously promotes good community relations Performs other duties as directed or required
REQUIREMENTS
Associates Degree in in psychology, social work, social science, public health or related field preferred (or equivalent combination of formal education and related experience may be substituted for degree) Prior experience working with at-risk populations including i.e., low income, mental health individuals and/or families Bilingual in Spanish/English preferred. Prior Housing search experience preferred Knowledge of city, state and federal housing policy and programs preferred Excellent communication, interpersonal, organizational and time management skills Cultural sensitivity and comfort with a wide range of social, racial and ethnic populations Basic computer knowledge, including Microsoft Outlook, Word, and Excel required. Ability to work independently. Strong organizational and problem-solving skills. Ability and willingness to give and accept team feedback; ability to communicate effectively orally and in writing. Ability to exercise good judgment and focus on detail as required by the job.
