Care Manager/ Protective Service Worker Indiana, PA 1 DAYS AGO 22764603 Summary Indiana, PA In-Person $20.52 per Hour 1 Years Experience Bachelor's degree No Commission 37.50 hours per week / Day Shift / Full-Time / Hours May Vary Description Job Summary The Care Manager is responsible for completing assessments for the OPTIONS and Care Giver Support Programs and investigating reports of abuse, neglect, exploitation, or self-neglect involving older adults and determining appropriate protective interventions for the Protective Services Program. Minimum Qualifications Bachelor's degree in social work, psychology, sociology, gerontology, human services or related field Valid driver's license and ability to travel within assigned service area Ability to pass required background checks and clearances and drug screening Key Responsibilities OPTIONS/Caregiver Support/Person-Centered Counseling Conduct person-centered assessments Develop, implement, and update care plans Coordinate service authorizations and linkages across ASI offerings, provider networks, and community resources Provide follow-up, monitoring, advocacy, and education to support stability, health, and consumer goals. Maintain regular contact with consumers/caregivers and service providers to track outcomes, resolve issues, and prevent service disruption Document assessments, care plans, case notes, and service activity in the Department of Aging's data system Use Department approved forms, terminology, and data entry practices referenced in the policy manual Protective Services Receive and screen reports of alleged abuse, neglect, exploitation, or abandonment of adults aged 60 and older (or as defined by state law) Conduct face-to-face investigations in private homes, healthcare facilities, and community settings Assess immediate safety risks and determine the need for emergency protective actions Interview older adults, caregivers, family members, service providers, and collateral contacts Perform comprehensive assessments of physical, mental, social, environmental, and financial conditions Determine consumer capacity and ability to make informed decisions Develop service plans that address safety concerns while supporting independence Coordinate and monitor services such as in-home care, medical services, housing supports, legal assistance, and mental health services Maintain ongoing contact with consumers to monitor progress and reassess risk Collaborate with multidisciplinary partners including law enforcement, healthcare professionals, legal entities, long-term care ombudsmen, and community service agencies Advocate for the rights, dignity, and choices of older adults Participate in case conferences, court proceedings, and mandatory reporting processes as required Maintain accurate, timely, and confidential case records in accordance with state regulations and agency policies Prepare reports, affidavits, and court documentation as needed Ensure compliance with all relevant laws, regulations, and professional standards Provide education to consumers, families, and community partners on abuse prevention, available resources, and protective services Participate in outreach activities to increase community awareness of older adult abuse and protective services Knowledge, Skills, and Abilities Working knowledge of Pennsylvania's aging programs (OPTIONS/Caregiver Support/Protective Services) Ability to coordinate services and benefits across community, healthcare, and social service systems; strong advocacy and consumer education skills Proficiency with electronic case records and detail-oriented documentation Excellent communication, cultural humility, interviewing and critical thinking skills, time management, and problem solving Working Conditions Combination of office and fieldwork, including home visits May involve exposure to challenging or unsafe environments On-call required and after-hours may be required Emotional demands associated with working with vulnerable populations. Physical Requirements Prolonged periods of sitting at a desk working on a computer Must be able to lift 25 pounds Must be able to operate basic office equipment (copier, fax, telephone, etc.) Additional Details
