Job Description

Progressive Rehab of PA, Inc., is looking to hire a Full-Time Case Manager in Milford, PA. Our mission is to empower people with acquired brain injuries to live their best lives. Progressive Rehab provides services to approximately 70 clients. The Case Manager is responsible for providing comprehensive counseling and case management services to clients served by the program. This role supports participants in identifying career interests, developing employment skills, and achieving goals while coordinating services and resources that promote independence and successful community reintegration.
Qualifications:
Experience in case management, rehabilitation, behavioral health, social services, or a related field preferred. Excellent verbal, written, and interpersonal communication skills. Strong organizational, time management, and multitasking abilities. Ability to think critically, solve problems, and make sound decisions. Ability to build positive relationships with clients, families, referral sources, and healthcare professionals. Proficiency in Microsoft Office and electronic medical record (EMR) systems. Ability to always maintain confidentiality and professionalism.
Responsibilities:
Develop and maintain collaborative relationships with clients, families, healthcare providers, referral sources, and community agencies. Assess client needs, strengths, preferences, and goals to assist in developing individualized treatment plans. Coordinate services among internal departments and external providers to ensure continuity of care. Participate as an active member of the interdisciplinary treatment team. Monitor client progress and communicate updates, concerns, and recommendations to the treatment team. Maintain accurate, timely, and compliant documentation in the EMR. Advocate for clients while ensuring services are delivered in accordance with organizational policies and applicable regulations. Maintain professional relationships while demonstrating compassion, empathy, and respect for all clients. Perform other duties as assigned.
