Full Job Description Job Summary:
The Senior Case Manager provides advanced case management services, leadership, and direct support to a team of shelter case managers. This role involves overseeing complex client cases, ensuring adherence to best practices, and actively participating in the training and development of team members. The Senior Case Manager is a critical resource for the case management team, offering guidance, problem-solving expertise, and ensuring high-quality service delivery to homeless families and individuals.
Essential Duties and Responsibilities:
Team Leadership & Support:
Provide direct supervision, mentorship, and guidance to a team of shelter case managers and shelter desk staff. Conduct regular one-on-one meetings and team huddles to review cases, discuss challenges, and ensure consistent application of case management principles. Assist case managers with complex client situations, including crisis intervention, de-escalation, and navigating challenging resource linkages. Participate in and lead case reviews regularly with case managers as well as the Shelter/Food Services and Housing Director. Facilitate training sessions for new and existing case managers and shelter staff on intake processes, goal setting, documentation, and best practices in an emergency shelter setting. Foster a collaborative and supportive team environment, promoting an attitude of cooperation and mutual support.
Advanced Case Management:
Manage a reduced caseload of complex or high-need clients, providing intensive advocacy and ongoing case management. Oversee the processing of initial intake assessments for new residents, ensuring thoroughness and accuracy. Guide clients in establishing and working on short and long-term goals related to housing, mental health, medical needs, and employment. Assist residents in identifying and accessing needed community resources to enhance their lives and promote independence. Ensure the accurate maintenance of resident files and comprehensive documentation of work performed by the team. Provide necessary referrals and advocacy for residents, including addressing the needs of children in assigned cases regarding health, education, and emotional well-being. Develop and review aftercare plans with clients as they prepare to exit shelter.
Program & Policy Adherence:
Ensure the department adhere to organizational policies, procedures, and ethical guidelines. Monitor compliance with grant requirements and reporting standards. Contribute to the development and refinement of case management protocols and best practices. Respect the confidentiality of records pertaining to residents and ensure team compliance. Lead/Support with shelter dorm searches, write-ups and disciplinary action as a result of searches. Conduct regular substance abuse groups weekly in compliance with grant requirements. Ensure chore compliance for all shelter clients. Participate in Director on Duty rotation to cover evening and weekend hours per a 24/7 operation.
Communication & Collaboration:
Communicate effectively with clients, team members, supervisors, and external service providers. Represent Opportunity House professionally in interactions with community partners and stakeholders. Attend and actively participate in staff meetings, inter-departmental meetings, and relevant training. Perform other job-related duties as assigned to support the overall operations of Opportunity House.
Qualifications:
Bachelor's degree in social services, social work, psychology, or a comparable field is preferred. A degree can be substituted for equivalent experience (e.g., 4+ years of direct, progressive experience in case management). Minimum of three (3) years of direct service experience in crisis intervention, case management, shelter/transitional housing, and/or substance abuse. Demonstrated leadership abilities and experience in mentoring or training staff. Excellent communication (written and verbal), interpersonal, and organizational skills. Strong problem-solving and decision-making abilities, with a focus on client and program best interests. Proficiency in basic computer knowledge, including Microsoft Office Suite and client management software. Ability to work independently, accurately, and efficiently, with an eye for detail. Self-motivated and able to multi-task effectively in a fast-paced environment. Ability to work effectively with individuals who have diverse styles, abilities, motivations, and backgrounds. Valid Pennsylvania Driver's License and good driving history. Ability to report to work in inclement weather. Ability to perform physical tasks as required (e.g., sitting, stooping, standing, walking, bending, carrying up to 30 pounds). Must be able to pass required background checks and clearances as mandated by state law and organizational policy for working with children.
Work Environment & Schedule:
This is a full-time, exempt position. Standard office hours apply, but flexibility may be required to meet client needs, attend meetings, or address emergencies. Work will primarily be performed in an office and shelter environment. Occasional travel within the local community for client advocacy, resource visits, or training may be required.
