Job Description

Job Summary We are seeking a dedicated and energetic Case Manager to connect with young people in our Teen Mentoring Programs, empowering youth to reach their full potential. In this vital role, you will develop strong relationships with adolescents, coordinate mentoring activities, and connect young people with essential resources. Your passion for youth development and commitment to positive behavioral support will foster a safe, nurturing environment where teens can thrive. Duties Build trusting relationships with adolescents, their families, and community partners to support their personal growth and well-being Develop and implement individualized case plans that address each teen's unique needs, including behavioral health and educational goals Coordinate mentoring sessions, youth development activities, and skill-building workshops aligned with adolescent development principles Provide ongoing client communication, offering guidance, encouragement, and conflict management as needed Connect youth and families with community resources such as childcare, special education services, social work support, and child welfare programs Maintain detailed case records using Microsoft Office tools, documenting progress and service plans accurately Support high-risk populations by enouraging PCES (Positive Childhood Experiences) Requirements Experience working with children and adolescents in mentoring or youth development programs Knowledge of behavioral health practices, child welfare systems, and special education needs Strong written communication skills for documentation and report writing Ability to work effectively with culturally diverse communities and underserved populations Experience working with individuals with disabilities or from high-risk backgrounds is highly desirable Familiarity with Microsoft Office applications for case management documentation Background in social work or related fields such as youth programs teaching or child development preferred Demonstrated conflict management skills and experience working with families in challenging situations Join us in shaping brighter futures for teens by providing compassionate support, mentorship, and essential services. Pay:
$25.00 - $28.00 per hour
