Job Description

I/DD Program Manager BAYADA Home Health Care has an immediate opening for a I/DD Program Manager in our Wilkes-Barre, PA office. Are you passionate about supporting individuals with intellectual and developmental disabilities (I/DD) and helping them achieve greater independence and quality of life? This could be an exciting opportunity to make a meaningful impact while growing your career in healthcare leadership. As a Program Manager you will serve as an advocate and internal case manager for clients receiving habilitation services. You'll work closely with clients, families, care teams, and community partners to ensure individualized goals are supported while providing leadership and guidance to field staff. What You'll Do Serve as an internal case manager and advocate for clients receiving habilitation services. Participate in Person-Centered Planning and support the development of individualized goals. Collaborate with clients, families, Case Managers, Habilitation Technicians, and interdisciplinary teams to implement and evaluate care plans. Conduct regular client visits to assess progress and ensure services align with the Individual Support Plan (ISP) and program requirements. Monitor and document progress toward individual goals and outcomes. Maintain strong communication with Case Managers, Care Coordinators, families, and other stakeholders. Supervise, coach, and support Habilitation Technicians in accordance with company policies and state regulations. Participate in client-specific orientations and training. Support client intake activities and travel locally as needed. What We're Looking For Bachelor's degree required. Experience working with the I/DD population required. Prior supervisory or leadership experience required. Demonstrated ability to take on increasing responsibility and achieve goals. Strong communication, organization, and relationship-building skills. Ability to collaborate effectively with clients, families, and interdisciplinary teams. Passion for helping individuals achieve greater independence and reach their personal goals. Why Join BAYADA?
