Care Manager Access Care Partners Pay:
$24.00-$27.00 per hour
Schedule:
Full-Time, 35 hours/week, Monday-Friday (8:00 AM-4:00 PM or 9:00 AM-5:00 PM)
Hybrid Opportunity:
Eligible for up to 2 remote days per week after successful completion of a 6-month introductory period. Make a Difference in Your Community Access Care Partners is seeking a compassionate and organized CTLP Care Manager to help nursing facility residents successfully transition back into community living. In this role, you'll work directly with residents, families, nursing facility staff, and community providers to assess needs, coordinate services, and develop person-centered plans that promote independence and quality of life. Key Responsibilities Conduct assessments and determine eligibility for community-based services. Develop and coordinate individualized transition and care plans. Educate residents and families about long-term care options and available resources. Coordinate housing, healthcare, financial, and support services. Participate in care planning meetings and interdisciplinary case reviews. Build relationships with skilled nursing facilities and community partners. Maintain accurate documentation and comply with program requirements. Qualifications Bachelor's degree in Human Services, Social Work, Psychology, Sociology, Healthcare Administration, or a related field preferred. Candidates with degrees in other disciplines and relevant experience or a strong interest in human services are encouraged to apply. Strong communication, organization, and problem-solving skills. Ability to work independently and as part of a collaborative team. Valid driver's license and reliable vehicle required. Bilingual candidates are encouraged to apply. Why Join Us?
