Job Description

Position Overview Hearts to Nourish Hope is seeking an experienced Case Manager & Data Specialist to provide client intake, case management, follow-up, and accurate HMIS/database documentation. The ideal candidate is organized, dependable, client-focused, and able to balance direct service with timely, accurate data entry. Key Responsibilities Conduct intake, assessments, eligibility screenings, case management, and client follow-up. Develop service/stabilization plans and connect clients with housing and supportive resources. Enter and maintain accurate HMIS and database records, including case notes, services, enrollments, exits, and outcomes. Review records for completeness and data quality and correct missing or inaccurate information. Coordinate with Hearts staff, housing providers, community partners, and other agencies while maintaining confidentiality and documentation standards. Required Qualifications 3-5 years of professional case management experience. 3-5 years of experience using databases, electronic client records, or data management systems. Strong computer, data-entry, documentation, organization, communication, and follow-up skills. Experience with client intake, assessments, case notes, and ongoing follow-up. Dependable attendance and ability to report to the Riverdale office as scheduled. Preferred Experience  HMIS and housing/homeless-services experience is a plus, including Coordinated Entry, Rapid Rehousing, Emergency Shelter, or Street Outreach.  Experience with grant-funded programs, data quality, reporting, or compliance documentation is a plus.
Important:
Starts at approximately 25 hours per week with potential to become full-time. This is an on-site position and is not remote, hybrid, or work-from-home.
Pay:
$18.00 - $20.00 per hour Expected hours: 20.0 - 25.0 per week People with a criminal record are encouraged to apply
