FULL-TIME
Pay:
CONTRACT POSITION WORK FROM HOME
with
TRAVEL REQUIRED COMPETITIVE PAY QUALIFICATIONS
Must be an LPN or RN with one year of paid professional experience or have a Baccalaureate degree with one year of paid professional experience with the aging or disabled population. Must be a certified Case Manager or will be required to complete one week of orientation including job shadowing a certified Case Manager before attending certification training Have valid driver's license and dependable transportation.
JOB DESCRIPTION
Assisting Advantage members to meet all their health and safety needs while allowing them to continue living in the comforts of their own homes. Educate, analyze, plan, coordinate and oversee service delivery. Evaluate member outcomes resulting from the care received. Identify member needs, goals and abilities Ensure ongoing coordination. Prepare and submit a comprehensive, written service plan for each member Implement certified ADvantage service plan Monitor delivery of services Identify high risk situations Orient and educate members Modify services to meet needs of members. Ensure confidentiality according to all relevant laws and regulations. Maintain adequate records Justifies expenditures and monitors and contains cost of plan. Assist in implantation of in-service education for field employees
SKILLS:
Excellent verbal, interpersonal and written communication skills; Excellent problem solving skills; Ability to use a computer and relevant data bases. Ability to organize and manage a caseload in accordance within agency standards and in compliance with policies and procedures of contracted programs.
