Job Description

About us We are professional, agile and our goal is to Primary Care Solutions provides an array of services to both adolescents and adults. Our mission is to provide housing, adaptive living skills, outpatient services, substance abuse services, supported employment services, and other necessary supports to youth and young adults who otherwise would be homeless, incarcerated, or institutionalized. When supported with the necessary services and support, every individual has the opportunity to live a healthy and productive life..

Columbus, Toledo and Cleveland Responsibilities:
  • Provide case management services to individuals and families in need
  • Conduct assessments to determine client needs and develop appropriate service plans
  • Collaborate with community resources to ensure clients receive necessary support and referrals
  • Assist clients in accessing childcare, healthcare, and other essential services
  • Utilize motivational interviewing techniques to empower clients and promote positive change
  • Monitor client progress and make necessary adjustments to service plans
  • Advocate for clients' rights and needs within the child welfare system
  • Supervise and support clients in their journey towards self-sufficiency
  • Provide workforce development assistance, including job readiness training and job search support
Skills:
  • Strong interpersonal and communication skills
  • Ability to build rapport and establish trusting relationships with clients
  • Knowledge of child welfare programs and services
  • Familiarity with Medicare, Section 8, and other public assistance programs
  • Experience in mentoring or coaching individuals towards personal growth and development
  • must have experience and have 5 clients that they bring when getting hired This position requires a compassionate individual who is dedicated to making a positive impact in the lives of others.
