Position Overview:
The Case Manager is responsible for assisting no less than 35 individuals receiving Homelessness Response Team (HRT) Services, in obtaining permanent housing, and addressing barriers to homelessness. This includes establishing and maintaining productive relationships with outside service providers, clients/client families, keeping case records/files, and ensuring that all activities are conducted in compliance with the funding source, program, and agency policies and procedures.
Essential Job Functions:
Manages client caseload assigned utilizing the EHR data platform. Models appropriate behavior and maintains appropriate boundaries. Conducts potential and current client assessments as/ when required Identifies significant family and community supports needed and develops a comprehensive, non-redundant Plan of Care (PoC) that utilizes the client's strengths to overcome barriers to care and encompasses the needs of the client and modifies/updates the plan as client circumstances and needs dictate. Ensures provision/obtainment of client services according to the PoC, including housing stability, financial stability, health promotion and education, prevention and wellness, social and peer supports, vocational needs legal issues, and behavioral health services/referrals, application for/obtainment of specific entitled benefits, employment search and development, and referral to community resources. Documents those activities in each client's case file. Involves the client's supports in the case management process as appropriate/ needed. Instructs clients in basic financial skills, including budgeting, savings, fiscal responsibility, and mechanisms to develop and build credit as well as the obtainment of benefits such as
SSDI/SSI.
Assists clients in obtaining services regarding their health concerns and needs and works with service providers to help the client meet those needs. Provides interpersonal intervention services to clients in the programs as needed. Maintains positive working relationships with community agencies and funding sources. Maintains positive working relationships both internal and external sources. Maintains appropriate contact with outside agencies contracted to provide specific services for clients, with Clinical Manager, functioning in an oversight capacity to assure client's involvement in the program. Maintains all required professional certifications and training and successfully complete other training required by regulation, funding source, or the agency and inherent to the position. Represents the agency in working with other community-based agencies to expand service networks. Participates in and contributes to other duties and responsibilities as assigned.
Education, Experience, and Qualifications:
Bachelor's degree in social work, sociology, public health, or nursing from an accredited four (4) year institution of higher learning is required. Combined five (5) years of education and experience in providing direct services in social, health, or behavior health fields; or Current Mental Health Rehabilitation Technician/Community (MHRT/C) Certification. CPR, First Aid, and Narcan certified. Skills and abilities in individual case planning and management, resource development, public relations, verbal and written communications, data entry, and group facilitation. Valid Maine driver's license, good driving record, and a reliable vehicle required. Strong comfort level and proficiency with technology, including Microsoft Office products
Working Conditions:
Must be able to work independently and as part of a team in both remote and office settings, maintaining regular communication with the HRT team and clients. The employee will be regularly required to sit, walk, stand; bend, reach with hands and arms; lift up to 25 lbs., use hands repetitively to operate standard office equipment; talk and hear, both in person and by telephone. Travel required to meetings, trainings, or other events.
