If the answer is yes, Echelon Community Services wants to meet you! We're looking for a dedicated case manager who is eager to impact our community. In this role, you'll earn a competitive salary of $60,000. You'll have the flexibility of working in our office just two days a week, up to two days at home, and you'll engage meaningfully with clients through fieldwork. And we offer a compressed scheduled. We're seeking individuals with excellent writing, computer, and organizational skills, as these are crucial for success. To be a strong candidate, you should possess a genuine interest in social services and have either paid or volunteer experience in case management or working with low-income families. Personal experiences related to homelessness are also highly valued, as they bring unique insights to this important work. If you are interested see job duties below and apply today for an interview tomorrow. Must provide case management services for 25 clients each month. Post 90 days of enrollment the case manager must conduct one face-to-face visit in the home for each client. The home visit must include a case management meeting and a home inspection. The first 90 days of enrollment there must to be four contacts per month for every client. Two face-to-face contacts which one must be in the home and two additional contacts to include telephone, text or email. During the home visit case manager shall conduct a home inspection. Must complete and submit monthly reports. Must enter all contacts or engagements into the HMIS system within 48 hours of contact. In HMIS must update and maintain case plan goals and action steps. Must have at least one goal. Must have one administrative day per pay period. Administrative day cannot include trainings, home visits, meetings or in office appointments. This time must be dedicated to administrative functions only. Remote position with the exception of required administrative day. Must have access to the internet and a quiet space to work without interruptions. Administrative day must include updating hard copy files located in the office. This information must include but no limited to case notes, resources, client documents, monthly documentation, intake forms and any other important information. Must meet monthly with the Director of the Program to review contract compliance. Must attend all trainings and meetings. Create and monitor an Individual Case Plan (ICP) for each client; Must be flexible and is able to meet clients based on their schedule. SPDAT assessment must be completed at intake and every 180 days. Provide life skills education to clients focused on the skills needed to support sustainability and independence to include financial planning, budgeting, obtaining and repairing credit reports, identification of educational, employment and training programs, household management and proper cleaning techniques and accessing community resources, etc. Prepare and submit Unusual Incident Reports via the electronic reporting format as needed to document client activity; Respond to crisis situations and assist with interventions to resolve immediate problems; Facilitate appropriate referrals for services as identified areas of need through client self reports or observation;

Must have Child Protective Clearance, local and state criminal background, FBI Degree preferred, or lived experience will replace having a degree. Drug and alcohol screen, TB Test, Sex Offender, First Aid and CPR certifications, Driving license and access to a car are required.Job Types:
