Job Description
IDD CASE MANAGER
- SLS/CES
Position Closes:
07/31/2026 The Case Manager is responsible for a variety of duties, which could include responding to contacts seeking information regarding services and supports for individuals with intellectual and developmental disabilities, assembling information about the individual for eligibility determination, providing Case Management, being responsible for the assessment planning, coordination, and monitoring of services and supports provided to individuals with intellectual/developmental disabilities and their families as appropriate. Spanish bilingual preferred with a $1.00 increase in pay rate.Requirements:
Bachelor's degree required; or five years of relevant experience in the field of Long-Term Services & Supports, which includes Developmental Disabilities. Driving is a requirement for this position. Prior case management and/or advocacy experience strongly preferred. Must be comfortable working with and near individuals with intellectual/developmental disabilities. Must display a high degree of responsibility, initiative, and interpersonal and communicative skills. - 29.75/hr. DOE/DOQ.
- Remote/Mobile position
- must be able to attend trainings and meetings in Fort Collins, CO and throughout Larimer County.
