This position combines elements of case management, care coordination, social work, disability services, long-term care, and community-based support to help participants maintain independence, achieve personal goals, and direct their own services. As an IRIS Consultant, you will build meaningful relationships with participants, coordinate services, promote self-determination, and ensure health and safety while supporting participants in living the life they choose.
