The Qualified Professional (QP) provides professional support, care coordination, and case management services to individuals receiving behavioral health and/or developmental disability services in the Gastonia, NC area. The QP works collaboratively with clients, families, healthcare providers, social workers, and community agencies to assess needs, develop and monitor person-centered service plans, coordinate appropriate services, and support clients in achieving their goals. The position requires accurate documentation, effective communication, advocacy, and compliance with agency, Medicaid, and North Carolina service requirements.

Responsibilities/ Job Duties:

Provide care coordination and case management for clients. Complete assessments, service plans, and treatment-related documentation . Monitor clients' progress toward identified goals. Coordinate services with families, social workers, physicians, therapists, schools, and community providers . Advocate for clients and help connect them with appropriate community resources. Participate in treatment team meetings and communicate updates regarding client needs. Maintain accurate, timely, and confidential clinical/service records . Provide or oversee skill-building, crisis intervention, and supportive services , depending on the program. Ensure services are provided according to the client's person-centered plan and applicable regulations. Follow agency policies, Medicaid requirements, and other applicable documentation/compliance standards. Assist with discharge planning and transition of services when appropriate. Respond to client or caregiver concerns and help resolve barriers to services.

Required Education/Experience:

Bachelor's degree in Social Work, Psychology, Counseling, Human Services, Sociology, or a related field from an accredited college or university. Relevant experience working with individuals with behavioral health, developmental disabilities, mental health, or substance use needs preferred. Experience with case management, care coordination, assessments, service planning, and documentation preferred. Must meet applicable Qualified Professional (QP) credentialing requirements for the state and service area. Why Work at Ethos?
