CASE MANAGER QUALIFICATIONS REQUIRED
Be at least 21 years of age Must have a minimum of a bachelor's degree in a human services field such as: Social Work, Psychology, Special Education, Education, Nursing, Counseling, Occupational Therapy, Speech Language Pathology, Art Therapy, Music Therapy, Recreation Therapy, Public Health, Addiction Counseling, Marriage and Family Therapy, Gerontology, Psychology, Human Services, Religion, or closely related fields. At least one year experience in serving persons with Intellectual and Developmental Disabilities (I/DD) or related disabilities in a case management or coordination type role
COMPETENCIES REQUIRED
Ability to operate a computer and perform basic functions for entering and maintaining records in a statewide information system Have and maintain a valid driver's license, have an automobile for use on the job, and maintain auto insurance Effective organizational and communication skills Willingness and ability to work flexible schedule
CASE MANAGER DUTIES AND RESPONSIBILITIES
Prepare, update, and monitor annual Individualized Service Plans Attend monthly staff meetings and trainings as directed Assist persons in identifying and accessing community resources Advocate for services and supports to meet the person's needs Understand and adhere to Agency Policy and Procedures Possess an understanding of waiver services available and processes by which those are obtained Compliance to privacy and confidentiality practices Complete all face to face and ancillary contacts required for caseload and complete documentation of contacts
