Summary We are seeking a dedicated Case Manager committed to assisting seniors and adults with disabilities in maintaining independence and remaining safely in their homes for as long as possible. Skilled in developing, implementing, and coordinating individualized plans of care while connecting clients to appropriate community resources and support services. This position is 37.5 hours per week with a pay rate of $22.00/hour. Westbay offers a generous benefits package including 3 weeks paid vacation, 10 sick days , 2 personal days per year and 14 paid holidays plus a paid day off on your birthday. Other benefits include life, health, dental, and vision insurance, and a 401k with company match. •Sign-on bonus of $2,000 will be paid according to the following schedule; $500 following successful completion of three months of employment, $500 following successful completion of six months of employment, and $1000.00 to be paid following successful completion of one year of employment. Only direct hires are eligible to receive the sign on bonus.•Qualifications Bachelor's degree in social work (B.S.W.) or a related field, or one (1) year of relevant professional or lived experience in case management, particularly with families or seniors Experience developing and implementing care plans required Pass a BCI clearance Valid Driver's License and reliable transportation required Must maintain appropriate auto insurance coverage Specific Duties The duties listed below are primary functions of the job, and are not intended to be all-inclusive. Duties, responsibilities, and activities may change, or new ones may be assigned at any time, with or without notice. Provide case management services, including initial contact with the client Conduct a client assessment and develop a care plan Provide follow-up support to seniors and disabled adults in home care or assisted living programs, including those designated by DHS, to ensure ongoing care, monitor needs, and coordinate services. Conduct home visits as needed Engage family and community support systems as available Provide supportive counseling and advocacy to clients and their families Coordinate services with other community resources Determine eligibility, needed resources, and aid in the application process Monitor clients and modify care plans as needed Provide information and referral services to telephone callers and clients of Westbay Community Action Maintain complete and accurate client records Maintain information needed for data collection and enter it into the agency's data system Prepare accurate reports and statistics as required Inform clients of their rights and responsibilities Support the client's efforts to have his/her voice heard and to get needed services Offer advocacy on the client's or family's behalf when needed services are not being adequately provided by an organization within the community's service delivery system Recognize potential conflicts of interest and refer consumers to the next appropriate service to ensure advocacy needs are met Participate in meetings and trainings as needed and in public policy forums when appropriate Educate the general community about senior services and Westbay Westbay Community Action, Inc. is an equal opportunity employer.
