Nashville, TN If you've ever finished a day at work and thought, "I like this work, but I wish there was more variety in my days," keep reading. The Triage Case Manager welcomes clients to Nashville CARES. You don't carry a set caseload. Instead, you're the person clients reach when something urgent comes up and their primary case manager isn't available, like a missed ride to a medical appointment, a food pantry referral, a question about benefits, a crisis that needs a steady voice on the other end of the line. You address a need and then you meet the next person. No two days are the same. This role is built for someone who gets energy from doing different things every day, like meeting new people, learning new situations, and solving a different kind of puzzle each time the phone rings. If you'd rather go deep with a small caseload over months and years, this probably isn't your role, and that's okay (though we have that position, too!). We're hiring for the person who thrives on variety. What You'll Do
  • Be the first point of contact when clients call in with urgent or unplanned needs, and help them figure out what happens next.
  • Provide comprehensive case management to people living with HIV, including connections to medical care, housing, nutrition, transportation, financial support, and community resources.
  • Meet with clients in the office, in their homes, or in the community.
  • Drive clients to appointments and services when transportation is the barrier between them and care.
  • Advocate with medical providers, other agencies, and inside the agency when a client needs someone in their corner.
  • Hand off cleanly to the primary case manager once the urgent piece is resolved, so nothing falls through the cracks.
  • Maintain clear documentation current so your colleagues can pick up where you left off.
  • Help maintain our client food store and pantry by stocking items and helping clients shop for their food.
Who Would Be a Perfect Fit for this
Role:
You probably do well in this position if: You like a fast pace and you like surprises. A quiet, predictable day feels long to you. You can hold warmth and boundaries at the same time. Clients feel cared for, and you go home at the end of the day. You're able to make complex things understandable to others. You can take something complicated and explain it so someone can actually use the information. You're resourceful. You'll dig around for an answer before you ask, and you know when to stop digging and ask for help. You keep track of things. Names, details, follow-ups, documentation and when you say you'll circle back, you circle back. You care about the work, about the people, about doing it well. What you'll need Bachelor's degree (Social Work or another Human Services field preferred; degree must be verifiable) Two years of community-based social or health care experience. A supervised BSW/MSW internship counts for one of those years.
Comfort with everyday technology:
Microsoft Office, Windows, a smartphone, electronic health records (we'll train you on ours) An ability to lift and carry up to 50 lbs to help with stocking in the store and pantry. Valid driver's license, a vehicle, and auto liability insurance — you'll be driving clients Who We Are

Pay Range:
$50,000-$60,000 - Positions are placed in tiers based on education, experience, and skillset, and this determines the specific pay rate of each employee.
