Job Description
Job Description The Veteran Housing Case Manager will:
- Facilitate the delivery of appropriate supportive services to Veterans and their families who are experiencing homelessness or at risk of becoming homeless.
- Conduct needs-based assessments, assist Veterans with connecting to public benefits, VA benefits, housing resources and other community resources as deemed appropriate.
- Create and implement housing stability plans, and work alongside a Case Manager Team Lead to ensure the Veteran household is receiving the services/supports they need.
- Assist the Veterans with identifying and applying for permanent housing.
- Act as an advocate for the Veterans with both landlords and other community partners.
Knowledge and Skills:
- Good written, oral and interpersonal communication skills
- Strong computer skills
- Strong time-management, prioritization and critical thinking skills
- Ability to work both independently and in a team-oriented, collaborative environment
- Fundamental understanding of Trauma informed care and a strengths-based service approach
- Ability to maintain accurate and detailed documentation regarding client progress and interactions
- Ability to be flexible and adaptive to unexpected challenges and changes
- Ability to present themselves professionally and maintain good working relationships with landlords and other community partners
Required Qualifications:
- High school diploma or GED
- Working knowledge of fundamental case management duties
- Able to take a rotating on-call phone in 1-week increments
- Clean motor vehicle record and background check
Preferred Qualifications:
- 1-2 years of case management experience with vulnerable populations
- Associate or Bachelor degree in Social Work, Social Sciences, Public Administration, Psychology or related field
- Veteran preferred
Compensation and Benefits:
- $21-23/hour -based off experience
- Paid training to become a Certified Case Manager (will receive a $0.50 pay increase upon completion)
