Job Description

TITLE:
Educational Support / Case Manager
REPORTS TO
Executive Director
JOB SCHEDULE
: PT Monday - Friday 3 pm to 8 pm
DUTIES AND RESPONSIBILITIES 1.
Meet with each family twice a week (at a minimum) assist them to identify the steps required to complete the goals assigned by the Family Advocate. Ensure that goals set at the weekly case management meeting are completed before the next meeting . 2. Create and maintain a database of services provided to each client including tracking their progress. 3. Provide psychoeducational support to clients to enhance their capacities and problem-solving skills. Educational support may include a variety of subjects including, but not restricted to parenting, stress management, anger management, mock interviews, etc. 4. Assist clients access community resources. Advocate for client services and maintain knowledge of other social agencies services in the community. 5. Collaborate with the ED to provide educational support to employees during the monthly mandatory staff meetings. Maintain a record of meetings using a goggle database and provide certificates for employees and for the employees' files. 6. Attend mandatory monthly staff meetings. 7. Provide aftercare services for shelter clients for a period of 2 years after clients have graduated from the shelter program. Reach out to clients by email on a weekly basis and by phone on a monthly basis to assess their progress and needs. 8. Maintain the aftercare database documenting client's interaction and progress. Provide psychoeducational classes for aftercare clients twice a month. Document classes information, attendees and all pertaining data. 9. Provide monthly reports on or before the 5th day of each month to the Family Advocate for input in the census report and to the ED. 10. Other duties as assigned by the Executive Director.
MINIMUM CREDENTIALS AND EXPERIENCE
Must have experience in case management (at least 2 years). Must pass the state background check. Must have a valid driver's license in Utah. Strong oral communication and interpersonal skills required to effectively communicate with a broad range of individuals and the ability to set good boundaries. Must have knowledge of Microsoft office suite. Ability to prioritize all aspects of the job, display professionalism and display leadership qualities. Must be able to work on a flexible schedule.

$18.00 per hour
