Provides direct mental health care to clients experiencing an acute crisis of a psychiatric nature that may jeopardize their current community living situation. Provides direct intensive services to clients, including linkage, advocating, supporting, and providing education as needed. Prepares written progress notes outlining the intensive services provided to clients in a timely fashion. Respond to crisis situations within twenty-four hours. Links clients to external programs or services, such as health services, recreational activities, childcare services, financial resources, employment resources, childcare, AA/NA groups, transportation resources, and others Collaborates and coordinates with other service providers and professionals, such as psychiatrist, therapist, medical doctors, attorneys, social workers, and probation/parole officers. Recommends appropriate services for client. Assures clients are informed of his/her human rights. Maintains contact with referring agency as needed. Conducts discharge planning in accordance with program description. Attends staff meetings, clinical training, and supervision meetings. Drives throughout the community to assess needs of mental health clients. Possesses physical strength to restrain clients if necessary. Maintains confidentiality of all client and employee information. Maintains excellent attendance to serve as a role model.
