Job Description

The pay for this position is $44,850-$58,909 annually. $500 sign-on bonus Sometimes it takes only one person to make a difference in a life. As the Community Case Manager for Adult Services, you could be that person. The Community Case Manager works directly with individuals receiving services to build functional skills, address barriers to care and social and mental wellbeing, and support follow-through on treatment plans and referrals. We want to hear from you if you have: An Associate's degree and two years experience and/or a bachelor's degree Knowledge on how to provide holistic and therapeutic case management services. Experience working in the mental health field. Computer and documentation skills. Ability to solve problems and deal with complex situations and stay calm.
