Job Description
The Backpack Case Manager develops and/or delivers ongoing community based assessment, treatment and supports for children and youth (clients) experiencing severe emotional disturbance. Supports will also be provided for families of the clients as needed to facilitate treatment of the client. High energy level, superior interpersonal skills and the ability to function in a team are required. All direct-service positions necessitate training and preparedness in physical restraint techniques, given the at-risk population served by the employer.
Essential Duties:
General:
Attend meetings as required by the Program Director, Laraway Youth & Family Services, and Local Educational Agency (LEA)
Attend trainings to increase knowledge and skills and as required
Develop, maintain and modify services to assist clients and their families in gaining access to needed medical, social, educational, vocational, housing, financial, and other services
Maintain contact with other professional service providers, LEA, LEA staff and others, for the purpose of case review or consultation regarding services to a specific client
Develop treatment plans to deliver, or provide for the delivery of, services identified by the treatment team
Work cooperatively with schools, social rehabilitative services and other agencies providing services to clients and family members of clients.
Fill in as school-based interventionist for clients when available
Maintain a positive teamwork approach with all other staff and consultants in providing services to the youth
Administrative:
Communicate behavioral data to appropriate treatment team members
Provide monthly case management and specialized Rehabilitation Summaries as required by Program Director
Provide assistance in the creating and scheduling of Backpack Summer Program(s) and other program activities
Other duties as assigned
Direct Services:
Provide continued communication with parents and/or legal guardian regarding services and treatment offered to the client.
Assist family members of clients in understanding, dealing with, and supporting client.
Provide social skill and anger management groups to specific clients in identified schools as directed by Program Director.
Assist in developing natural support networks for clients and family members of clients with severe emotional behavioral disturbances.
Assist clients and family members of clients in accessing community resources. Determine appropriate course of treatment for interventions.
Other Duties (Supervisory Responsibilities):
Collect data forms completed by interventionist for the purpose of documentation, progress and treatment plan goals.
Provide ongoing training and support to staff in Handle With Care™ techniques and protocols.
Problem solve and manage crises
Coordinate interventions and treatment with treatment team
