We are seeking a dedicated and compassionate Case Manager to join our Intensive Out Patient (IOP) team. As a Case Manager, you will play a vital role in providing support and guidance to clients in need. This is a 5 day, 8 hours/day position Starting wage for this position is $24.20/hr, and includes 100% agency-paid benefits. This wage is for a minimally qualified candidate

• Additional years of applicable experience will raise the starting wage offer accordingly.

Responsibilities:

• Must be 21 years of age or older.
• Conduct needs analysis with clients and develop individualized care plans
• Collaborate with healthcare professionals to coordinate and facilitate client care
• Provide education and support to clients
• Monitor client progress and adjust care plans as needed
• Assist with care coordination and ensure appropriate follow-up care
• Advocate for client's rights and ensure they receive necessary resources and services
• Maintain accurate and confidential client records in compliance with HIPAA regulations
• Utilize case management software to document client interactions and track progress
• Maintain a minimum productivity ratio of 40%

Experience:

• Bachelor's in social work or related field
• Previous experience in case management, preferably in a healthcare setting
• Knowledge of behavioral health, and addiction counseling
• Familiarity with utilization management principles
• Strong communication skills, both written and verbal
• Ability to work effectively as part of a multidisciplinary team
