Job Description
We are seeking a dedicated and compassionate Case Manager to join our Intensive Out Patient (IOP) team. As a Case Manager, you will play a vital role in providing support and guidance to clients in need. If you have experience in social work, behavioral health, and case management, we encourage you to apply. This is an excellent opportunity to make a difference in the lives of individuals and families. This is a 5 day, 8 hours/day position Starting wage for this position is $24.20/hr, and includes 100% agency-paid benefits. This wage is for a minimally qualified candidate
- Additional years of applicable experience will raise the starting wage offer accordingly.
Responsibilities:
- Must be 21 years of age or older.
- Conduct needs analysis with clients and develop individualized care plans
- Collaborate with healthcare professionals to coordinate and facilitate client care
- Provide education and support to clients
- Monitor client progress and adjust care plans as needed
- Assist with care coordination and ensure appropriate follow-up care
- Advocate for client's rights and ensure they receive necessary resources and services
- Maintain accurate and confidential client records in compliance with HIPAA regulations
- Utilize case management software to document client interactions and track progress
- Maintain a minimum productivity ratio of 40%
Experience:
- Bachelor's in social work or related field
- Previous experience in case management, preferably in a healthcare setting
- Knowledge of behavioral health, and addiction counseling
- Familiarity with utilization management principles
- Strong communication skills, both written and verbal
- Ability to work effectively as part of a multidisciplinary team
