A Case Manager coordinates delivery of a range of social and health services to individuals and families. Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs.

Job Description
The success of DHR is dependent upon our providing the public with the highest possible level of service. DHR is a peer ran agency offering equal employment to those in recovery while valuing lived experience as a strength. Job includes but not limited to
- Providing supported housing and employment services including, but not limited to: housing and employment assessments, identifying housing or employment resources, support obtaining a lease, independent living skills development, landlord relations, crisis management, creating resumes, jobs skills coaching, etc.
- Knowledge of social work principles and practices
- Familiarity with workforce development programs and resources
- Understanding of behavioral health issues and the challenges people face
- Knowledge of housing assistance programs is a plus
- Strong leadership skills with the ability to inspire and motivate others
- Excellent communication and interpersonal skills
