PAY RANGE

$42-45/hr

Responsibilities:

Manage chronic/complex-care members by conducting assessments and developing individualized care plans.

Coordinate and monitor care, document progress, and communicate with members/providers, including home or face-to-face visits as needed.

Qualifications:

RN license (WA or Compact)

3-5 years Case Management/Insurance CM experience

Quality Gap Closure preferred
