Job Description

Job Summary Due to the internal promotion of a staff member and a growing caseload, an HIV case manager position is available in the Morgantown, WV area with the AIDS Task Force, a non-profit agency that is a leader in providing quality comprehensive services to HIV infected West Virginians. The AIDS Task Force, established in 1987, provides direct services, including medical case management to West Virginians living with
HIV/AIDS
through the Ryan White Part B Program. The ideal candidate for this full- time salaried position will have a minimum of a Bachelors Degree in Social Work with a curre4nt license or a Current Nursing License . Candidates should be energetic, self-motivated, able to work as a team of service providers, have a caring attitude and possess excellent communication, organizational and inter personal skills. All applicants must have a valid driver's license and have dependable transportation. A current WV Social Work license or nursing license is absolutely required. The successful applicant for the HIV Case Manager will perform intakes, assessments and work with HIV infected clients to assist in accessing and maintaining a continuum of care that improves their health outcomes. Duties include coordination of medical treatment, initial and ongoing assessments, treatment adherence counseling, creating and monitoring individual service plans and applying for entitlement programs. In this vital role, you will coordinate comprehensive care and support services for individuals living with HIV, ensuring they receive the necessary medical, social, and mental health resources to improve their quality of life. Your expertise will help bridge gaps in care, promote adherence to treatment plans, and empower clients to achieve optimal health outcomes. This position offers an exciting opportunity to make a meaningful impact in the community by providing personalized case management within a collaborative healthcare environment. Responsibilities Conduct thorough intake assessments to evaluate clients' medical, social, and behavioral health needs. Develop individualized care plans tailored to each client's unique circumstances and treatment goals. Coordinate medical appointments, medication management, and follow-up care in collaboration with healthcare providers. Provide education on HIV management, medication adherence, and risk reduction strategies. Facilitate access to social services such as housing, transportation, mental health support, and substance use treatment. Maintain accurate documentation of client interactions and care plans within electronic health record (EHR) systems while adhering to HIPAA regulations. Monitor client progress through regular check-ins, adjusting care strategies as needed to promote engagement and retention in care. Skills. Strong nursing background with knowledge of medical terminology Knowledge of social work principles, behavioral health, and working with diverse cultural backgrounds enhances client engagement.
