You will be train to assess disabled patient home for grab bar, ramp and mobility needs for bathroom. Assess asthma patient home for allergic source and teach patients about asthma prevention. You are also an In-Person Welfare Check and outreach coordinator, you will conduct home visits to check on patients' well-being, connect them with their Lead Care Manager and occasionally pick up records from primary care office.

Qualifications:

Bilingual in English and Spanish required Valid driver's license and excellent driving record Strong communication and people skills Comfortable using measuring tape. Records keeping, using smartphones and basic mobile apps Ability to work independently and manage a dynamic field-based schedule Passion for improving care access for vulnerable populations
