Exodus Recovery, Inc. is looking to hire a full-time Community Re-engagement Navigator for our SaMo Bridge program in Santa Monica, CA.

POSITION SUMMARY

The Community Reengagement Coordinator will work with a multidisciplinary team to provide outreach and engagement efforts to existing clients in the community that require re-engagement to program services. Additionally, The Community Reengagement Navigator, works collaboratively with law enforcement and community partners to reconnect eligible individuals experiencing homelessness or behavioral health challenges with supportive services provided thru SAMO Bridge Program. The CRN's responsibilities include participating in ride-alongs with SMDP officers to engage individuals who have previously declined services or completed a one-time intervention through SAMO Bridge, as well as coordinating with the Culver City UCC to identify and transporting eligible clients to SAMO Bridge.

MINIMUM QUALIFICATIONS AND EDUCATION

Extensive lived experience and/or professional experience in working with complex, homeless populations who are high users of multiple public health systems. Two (2) years clinical experience in health sciences or related field. Strong verbal and written communication skills.

EDUCATION

High School Graduate or GED equivalent. Valid California driver's license and a good driving record is required. Must be 21 or over to drive
