Job Overview The Outreach Coordinator is responsible for conducting home and community visits to further engage participants, streamline intake paperwork, connect families to recommended care coordination and treatment services, and provide referrals to essential resources such as benefits, food access, and harm reduction supports. Additionally, this position would play a key role in community outreach events, increasing awareness of Inspire to Rise, Inc.'s mission and services while also distributing harm reduction resources and concrete supports. Responsibilities Conduct home and community visits to help engage referred participants. Assist with completing intake paperwork with referred participants as needed, including participants in jail. Assist participants with linkage to care coordination and treatment services. Provide concrete supports and referrals to essential resources such as benefits, food access, and harm reduction supports. Research educational and outreach material for participants. Develop and implement outreach strategies to engage the community. Represent agency in community and/or interagency activities. Build community partnerships with local organizations, businesses, and stakeholders. Help organize community events, meetings, workshops, and informational sessions. Follow up with prospective participants that expressed interest during community outreach events. Provide evidence-based parenting curriculum, such as Nurturing Parenting Program, services to educate parents and address family needs. Will serve as backup to Community Service Coordinator team as needs arise. Requirements Bachelors Degree required, in Human services field preferred 1-year experience in community engagement, outreach, or related fields. Some experience working with at risk populations.
