grant. This role connects individuals with opioid use disorder or polysubstance use disorder to overdose prevention resources, treatment, Medication Assisted Recovery (MAR), and community-based support, with priority given to individuals in housing transition, exiting incarceration, or exiting inpatient treatment. You'll build the healthcare, criminal justice, and housing partnerships that make warm handoffs possible across our 10-county service area. This is a field-based, individual-contributor role. You will not be providing clinical services, this is peer support, outreach, and care coordination.

What We Offer Compensation:

$18.00-$23.00 per hour, non-exempt (overtime-eligible)

Meaningful Work:

Direct, hands-on impact on overdose prevention and recovery outcomes across LaSalle County and WARM's broader service area Key Responsibilities Conduct outreach at county jails, hospital emergency departments, inpatient treatment centers, homeless shelters and encampments, housing programs, recovery residences, and law enforcement/probation/parole offices Identify and engage individuals in the grant's priority population, including those pending or recently discharged from jail, prison, hospital, or inpatient treatment Provide peer support services, including emotional support and practical support adjusting to new housing Offer naloxone kits and overdose education to every individual contacted through outreach Conduct active, participant-present transfers (warm handoffs) to MAR, mental health, SUD treatment, housing, and recovery-support services Arrange or provide transportation and, when appropriate, accompaniment to initial appointments Build and maintain partnerships with healthcare, criminal justice, and housing/social service organizations Represent WARM at community events and resource fairs, including setting up and staffing WARM's resource table Maintain accurate documentation of all outreach, naloxone distribution, warm handoff, and housing activity for grant reporting Assist participants with housing search, subsidized housing applications, and Coordinated Entry applications Qualifications Required Valid Illinois driver's license and reliable personal transportation Ability to travel locally up to approximately 50% of work time Strong relationship-building, verbal, and written communication skills, including comfort with public presentations Proficiency in Microsoft Office and standard documentation systems Ability to work a flexible schedule, including some evenings Ability to lift/move up to 25 pounds and work in varied field settings and weather conditions Preferred Qualifications Active CRSS (Certified Recovery Support Specialist) or CPRS (Certified Peer Recovery Specialist) credential, or actively working toward one Personal or professional lived recovery experience 2+ years' experience in behavioral health, substance use disorder treatment, or related human services Knowledge of LaSalle County and WARM's broader 10-county service area Working knowledge of HIPAA and 42 CFR Part 2 confidentiality requirements Bachelor's degree Why Join WARM You'll be on the front line of overdose prevention, connecting people at their most vulnerable moments, exiting jail, exiting treatment, or in housing crisis, to the resources that keep them alive and moving toward recovery.
