In this role, you will help connect individuals and families with valuable healthcare and support services within the community. This position is ideal for someone who enjoys networking, helping others, building relationships, and making a positive impact. As a Patient Outreach Specialist, you will work closely with local community members, support groups, organizations, and families to help refer new patients to our services.
