Hybrid - Central Minnesota/St. Cloud area Carivida Place LLC is seeking a friendly, professional, and outgoing Community Outreach & Referral Coordinator to help build relationships with healthcare and community referral partners and connect with individuals and families seeking assisted living services. This position is ideal for someone who genuinely enjoys talking with people, building professional relationships, and making meaningful community connections. Responsibilities Reach out to hospitals, clinics, rehabilitation centers, social workers, case managers, discharge planners, and other referral partners. Introduce Carivida Place and develop ongoing professional relationships with community partners. Communicate with prospective residents and families in a warm, compassionate, and welcoming manner. Follow up with referral sources and prospective residents. Conduct outreach through phone calls, emails, networking, and occasional in-person visits. Maintain organized records of contacts, referrals, and follow-up activities. Professionally represent Carivida Place within the community. Qualifications & Skills High school diploma or equivalent required. Experience in healthcare, assisted living, marketing, outreach, admissions, sales, customer service, or a related field preferred. Excellent verbal communication and interpersonal skills. Confident and comfortable initiating conversations with healthcare professionals and community partners. Strong networking and relationship-building abilities. Warm, compassionate, professional, and approachable personality. Dependable, organized, self-motivated, and able to work independently. Basic computer and email skills. Knowledge of assisted living, healthcare, senior services, or community resources is a plus. Existing professional connections within Central Minnesota healthcare or human services are highly desirable. Location Requirement Applicants must reside in Minnesota , with preference given to candidates in or near Central Minnesota, including St. Cloud, Sartell, Sauk Rapids, St. Joseph, Waite Park, and surrounding communities.
